he will only appear after a Sim has died.

buy the best, most expensive refrigerator, then gather the following ingredients:

llama, squid, eel, fauxlestra.

make it into something or leave it as is and have your Sim eat it.  they will throw up, wet themselves, then die.  the Reaper can then be found outside the house.

He comes when somebody dies. Start a fire and watch your sims be carried away to death.
