  1. Depends on the type of aircraft.  Most carry the majority in the wings however long range aircraft such as 747, 777 and 757's have fuselage tanks and 747-400 has a 2800 gallon tank in the horizontal stabilizer.

    In smaller airplanes like Cessna 172's each wing has a fuel cap, just like a car, and each tank (in the wing) is filled up.

    On transport aircraft like EMB-145's, 737's and 777 they have what is called a single point fueling point.  The fuel truck pulls up, and hooks up to a single point.  And pumps all the fuel in, and the internal "plumbing" of the fuel system routes the fuel to the various tanks.  Before the fueler starts he/she enters the total fuel requested and the single point fuel system will stop accepting fuel once the requested ammount is delivered.

  4. Usually there is a separate filler for each tank.  On smaller airplanes, that's all there is to it.  Many larger transport airplanes also have single-point fueling systems that have a single pressurized filling point that fills all the tanks through a network of internal pipes.
