If you are talking about the hand as a whole, I think the fulcrum will be the wrist.  The hand is an example of a third order lever (the effort (muscles) being between the fulcrum and the load (what you are holding).  However, the hand is a complex lever, with there being a fulcrum at every joint.

    A better example of a third order lever is the forearm, with the elbow being the fulcrum, and the effort obviously being between the fulcrum and the load.
