  1. Don't even have to get a specific one I do not think.   So long as it is dated.   Probably from city hall.


  2. There is no set way to do it.  Here is a sample.

    TENANT’S NOTICE TO VACATE

    To: ____________________________________ Date: __________________ Please take notice of our intention to vacate our residence located at ________________________________________... on or before ________________________________________... The reason we are moving is : ________________________________________... We understand that our deposit will be refunded as agreed, less past due unpaid charges, if any, after we have moved out completely and returned possession of the premises to the management, as long as we leave the residence in clean and undamaged condition. We understand that our Lease/Rental Agreement states that we have agreed to a ________ day written notice to vacate.
