  1. If you mean the first train to be pulled by a locomotive, this was the one pulled by the steam engine 'Pen-y-Darren' near Merthyr Tydfil in Wales, built by Richard Trevithick in 1804, at the local ironworks.

  4. Well, are you referring to a steam-powered engine train, or just a train on rails? Because before the steam engine, there were horse-drawn railroads which pulled multiple coaches on parallel rails. Somewhere in Europe is where the first of these occurred, I believe. Then when steam power came along, someone got the idea to apply it to the rail system.
