Where would I go to take an old navy survey? 8/28/08?

Its August 28th 2008. I have the receipt and all but i need the survey address.

The address is on the survey slip that you received WITH your receipt - your receipt is not the survey and will not allow you to get 10% off just by writing the code on your receipt - you have to have the survey slip for the discount.

Please try & take the survey! The stores really need the feedback. You'd be surprised that with a few HUNDRED surveys given out PER DAY, the store only gets about 50-60 respondants per MONTH.

It only takes 10 minutes to complete.

Transaction #, Date, Store # is all you need to start the survey, so hopefully you can read those on the survey slip.
