  1. Start with the insurance company who is the fraud victim.


  2. I would start with your state's department of insurance.

  3. The state department is good. If you know the company which is impacted, even better. In either case be prepared to give a statement including your contact information.

  4. Report the matter to the state insurance commissoners office.  Depending on how much you know, they may have an investigator get in contact with you.  If the I.C. office can't help you, then they should lead you in the proper direction.

  5. There's no federal oversight.  You have to report it with your state insurance department.  Many have a website which allows you to report over the internet - some will require you to print out the form and mail it in.
