  4. I can't find anyone who meets the exact question but I've found two Lancashire cricketers who won a FA Cup medals and they are Harry Makepeace and Jack Sharp. Both played for Everton in the 1906 Final in which they won 1-0, however neither scored.

Everton also reached the final the following year with both players again appearing, Everton lost 2-1 but the scorer of Everton's goal was Sharp.

In a 26 year career for Lancashire Sharp played 518 matches scoring over 22,000 runs.

  6. Jack Dyson - Spent four seasons with Manchester City during which time he played 62 matches and kicked 26 goals. One of those goals came in their 1956 FA Cup Final victory over Birmingham.

Later on went on to play for Lancashire.
