Unless it's deemed medically necessary, insurers won't cover it at all.

It depends on the person and the health care plan really.

Go see a few PS's and have them submit information and see if you need to do anything else besides submit information.

I answered this in your other post.  However, I wanted to mention that in some states if you do not have prior coverage, a breast reduction can be excluded as pre existing.

Additional info:  In some states, if you don't have 18 months of prior coverage, carriers will exclude any health issues for up to 12 months.  Without knowing what state you are in I can't tell you for sure.
