  1. The second one.  You would have to separate the two because the word "question" is used and not "questions".  So you would say "The answer to question #5 is to read yes" and "The answer to question #6 is to read yes". Therefore "The answer to question #5 and #6 is to read yes".  If the word "questions" was used you'd go with the first option.  **The person below me is right.  They both sound screwy but the second one is less screwy


  2. They both sound wrong to me, the subject-verb agreement is off. I think that this is better:

    The answers to questions 5 and 6 are to read yes.
