Which is grammatically correct?

Please explain why or why not these are correct.

The second one.  You would have to separate the two because the word "question" is used and not "questions".  So you would say "The answer to question #5 is to read yes" and "The answer to question #6 is to read yes". Therefore "The answer to question #5 and #6 is to read yes".  If the word "questions" was used you'd go with the first option.  **The person below me is right.  They both sound screwy but the second one is less screwy

They both sound wrong to me, the subject-verb agreement is off. I think that this is better:

The answers to questions 5 and 6 are to read yes.
