Which number is the policy number on my medical insurance card?

On my card, there is an identification number (12 digits long, beginning with three letters), a group number (8 digits long), and a plan number (3 digits long). Which do I use when asked for the "policy number"?

  1. The ID number that's 12 digits long - and give them the prefix (the letters). Keep the group number and plan number handy though - they may ask for it.
