As far as looks:

obviously the face, neck, hands, anywhere thats visible and "reglular people" think is too much

Pain:

The inner lip, the tips of toes and bottoms of feet, the armpit, buttcrack, genitals, the areola, the gums, EYEBALL!!

Well I can think of some very extreme places, (a friend of a friend is a tattoo artist)

Inside lip, Clitoris (why oh why?!?), p***s (again WHY?!?) Places like that, I'd imagine it bloody hurts. Well that goes without saying really. In between toes and fingers, armpits (hmmm) and my personal favourite eyelids. Inside and out, is what I'm told. But I doubt it's on the inside.

Btw: Kebert Xela I think Scruffy means eyelash line, right on the edge of your eye. You can get it done I believe, but I'm not too sure =S

well, there's a couple levels of extreme in my book

hands/face/goodies (genitals) are pretty extreme

but then you have the eyes (sclera).....that one takes the cake for me
