my brother has a high white blood cell count and is on pain tablets for injury to legs.

White blood cells (leukocytes) are intended to fight infection.  One reason for an elevated WBC count is the presence of an infection.  The body has increased the number of "fighters" in the "battle".   2. 99% likely to mean an infection.  1% likely to be a chronic medical problem like leukaemia.  If infection, the infected area is likely to be weeping fluid, smelly and hot to touch
