White lumps under lips? What?

Question

My fiance has these tiny white bumps clustered together under his lips. They are really tiny, I can only see them when I'm right against is face but they kinda look like pimples. He's healthy but he does smoke alot and he used to be a heavy drinker until a year ago.

treat like herpes.  avoid them and get him to a doctor.
