There were more than 5 names that had death warrants issued:

Death Warrant for Sarah Good, Rebecca Nurse, Susannah Martin, Elizabeth Howe, and Sarah Wilds.

However there were several more than 5 accused and found guilty of witchcraft...37.  Some died in prison.  See below:
