  1. Depending on the child's situation usually the teacher.

    If the child is in special education then possibly the homeroom teacher, special education teacher (aka intervention specialist), therapists (speech, occupational and/or physical), social worker/advocate and the parents (who have final say)


  2. It depends.  Typically the school psychologist will do intelligence testing, behavioral checklists, etc.  While the special education teacher will do achievement testing (reading, writing, math, etc.)  Speech language pathologist will do language and auditory processing.

Usually there is a team involved in special education assessment that includes the school psychologist, special education teacher, student's general education teacher, school social workers, and specialists (like speech language pathologist, occupational therapist, autism specialist) that are involved as needed. The school principal typically attends meetings, but does not collect assessment information. Parents also should play an important role on the team as they know the child the best and can provide useful information about the child's history and how they behave at home.

    As stated above, the school psych usually does intelligence tests (as needed), behavior checklists, and observations while special education teacher adminsters achievement tests. File reviews and interviews of the student, teacher(s) and parent(s) should also be done to get a broad view of the student.
