Question:

Who do I go to for a stage name?

Besides making it up by myself, who would do it? i.e. manager, record label rep, etc?

No... you just do it yourself.  Choosing an appropriate name.. the one that you will present yourself with,  should be no one's responsibilty but yours.   You must know what names you like...   then think of the last name that blends with it nicely.   For example.  My daughter's name is Angie Stone...   we were tossing around what names would blend together and I thought what sounded good was Angela Langell... I think it just goes together.   You need someone to brainstorm with and toss around names that you like.  If the other person/people feel it matches you, your personality and stands out from the plain regular names... then you have the positive feedback to go with it.   Don't let some stranger slap you with names they think are good... they don't know you and besides that, once you get going you will have to live with it.
