  1. when I prepare my cover letters , I always put " To whom it may concern:" This takes the guess work out of  who it goes to.  even if you have the employer information, it may go to someone else.  I've never had a problem during an interview, and have always recieved compliments on my resumes. Please email me if u need more info.

  2. Direct it to "Human Resources Director"

  3. yes you can direct your letter to that person name

  4. If there is a name at in the advertisement, then that's probably whom you should address your letter to.

If you're not sure, though, try calling the main number at the company and asking to whom the letter should be directed.  It's always better to target a real name than a generic title, if you can avoid it.
