    Pat Welsh (Birth name: Patricia A. Carroll February 11, 1915, San Francisco, California - d. January 26, 1995 in Green Valley, California.) was a movie actress who appeared only in three movies, most famously as the voice of E.T. in "E.T. the Extra-Terrestrial". When Welsh was in a camera store, Ben Burtt, the voice designer of the character E.T., heard her talking and asked to audition for the E.T. character. The reason for her raspy voice is that she has been a chain smoker, and giving E.T.'s trademark speech sound. Welsh died in 1995 from pneumonia.

    Waterloo Bridge (1940) (uncredited)

    It's been said that Debra Winger helped out. IMDb has this:

    Debra Winger not only provided the temp voice for E.T. but also played one of the ghouls in the Halloween sequence. She is wearing a monster mask and a lab coat and carries a poodle.

    I also found this:

    Ben Burtt overheard her talking in a camera store and asked her to audition for the voice of "E.T.: The Extra-Terrestrial" (1982); chain-smoking led to her raspy voice, which was used simultaneously with Debra Winger's. Welsh was originally paid $380 for 9 1/2 hours work.

    I know that's not an absolutely definite answer since it names two actresses. Perhaps Welsh is the main one, with Winger supplementing.
