Who fills out my child immunization card?

my daughter got some shots today and i was wondering if i am suppost to fill out the immunization card or if i was suppose to have the doctor do it cuz i forgot to ask

The doctor should fill it out.  If you get shots at the health department - they will fill it out.  The next time you go back or if in the area - take the shot record in and ask the doctor's office to update it.

your doctor is supposed too. its never to late to go back to the doctors office and ask them to fill the card out. they will just go through the child's records and fill it in. good luck!

The nurse in my Dr's office always did.  Just get them to update it next time you are in.
