You do if it is for college.  They will ask every aspect of your parents financial business, even stuff, you as their child shouldn't have to know.

you fill it out. if you are under 18 and live at home you parents income will be considered

From what I recall, you fill it out, but you need a LOT of information from your parents.  To make it easier, I would fill out all of the information that you can, save it, then pull it up on your parents computer to finish it.
