Who gets charged with three way calling?

I am going to use false names:

If Jack and Jill are on the phone, and Jack calls Ben (Who is in the country) and then connects him to Jill, so they are three way: Does Jack get charged, or does EVERYone get charged?

  2. Since Jack made both calls he would be the one charged.

    If Jill was, say in Atlanta & she called Jack in Cleveland & he then called Ben In New York, Jill would be billed for the call the Cleveland, Jack would be billed for the call to New York & Ben gets off scott free.

    Jack would also have to have the 3 way calling on his line of course(monthly or pay per use).
