  1. Generally it's the governor of the state the representative is from, but they don't have the authority to remove someone from office.  An interim appointment is only made after the rep leaves office, for whatever reason.

  2. Good question!  I believe the governor can appoint a replacement for a Senator who dies or resigns before his or her term is up.  For the House, there has to be a special election.

  3. The governor of the members state and a special elections.

  4. They have to be impeached just like the president. Each house of congress has a committee that deals with their members and if it is deemed they did something wrong, then a vote can be brought before that house of congress to remove them from office.

  5. If they don't lose their election, they have to be impeached or die.  I believe they can be recalled by a special election in their home state.  If they are Republican, all they have to do is be caught violating the Congressional ethics.  If they are a Democrat, that same standard does NOT apply.

Once they have been removed from office, by whatever means appropriate, it is up to the state which they represented to name a successor.  This can be done by special election, or an appointment by the governor of the state.  Sometimes, if it is close to an election, the seat can remain vacant for a short period of time.

  6. If Senator dies, resigns, takes higher office or is impeached the governor names a replacement till next election. If Representative, dies, resigns, takes higher office or is expelled by House, it is iffy if special election is held or they wait till next election.
