  1. The executor or those acting for him, At the latest upon distribution.

  2. The executor.  If it's a fixed bequest and there's plenty of money to cover it, as soon as possible after probate.  If it's a residual bequest, as soon as possible after the estate has been settled.

  3. Executor.

NB. There are multiple scams on the web, typically seen via unsolicited eMails, in which you are 'informed' by a 'solicitor' (or, more typically, a 'Lawyer') that some rich relative (you have never heard of) has died in Nairobi and left you $1 million and all you have to do is send them (the Nigerian con-merchant) £10,000 in 'death duties' (ha ha) ...

  4. Either a lawyer or the executer (sp?)of the estate should notify you.  I don't know if there is a time limit or what that may be.

  5. This is a legal question and not an insurance question but I would've thought the executor.

  6. solicitor or executor of the will, when the will becomes active (when he person who's will it is passes)
