  1. Good question, but maybe not answerable.

    As you probably know, the first Test match was in 1877, but records of those early matches are not as comprehensive as they are today.

    A quick look through CricInfo, gave me this match in 1902:

    CB Fry was out for 0 but the record doesn't indicate which ball it was!

  3. Improbable task. Let me list out the possibilities:-

    Archie MacLaren was out first ball of first innings in this match in 1894.

    & WG Grace (2nd inns of the match, first inns of Eng, in 1890)

    It is certain that MacLaren & Grace lost their wicket, first ball in the respective innings, but can't be sure whether it was first instance of such occurrence in Test cricket.

    These are a few matches where a wicket fell possibly in the first ball, can't be sure

    Did you analyse the scorecard carefully?

    CB Fry was the non striker (Maclaren was the striker), & Hopkins was the the second bowler with Jones opening the bowling. So 0/1 means that the first over was a maiden, and Fry lost his wicket possibly in the second over.
