According to a recent FBI report the Italian Mafia is still strong along the East Coast despite growing Asian, East European and Russian Organised Crime Factions infiltrating the US and recent FBI Anti Mafia Operations.. The FBI also reported that crime rackets are now much more global.

Worst crimes? The current federal administration. Although they aren't that organized but they are venal and brutal. They've caused the deaths of thousands  America's finest and suspended much of our Constitution just to prove themselves right and show up daddy.

In the old style sense of using street violence and threat for financial gain I'd have to say the Latin Kings.

Most of the old ones are now semi legitimate businesses. For example the largest trash company in the US was started by what was called Mafioso.
