medical cannabis is provided through many growers who are registered by the state as caregivers and sell directly to the dispensers. In most cases patients can grow thier own according to county guidlines and can be as much as 100 plants in some instances. There is no true top ten of manufactures because even tho they have the permission by the state to provide for the clubs, some of the cannibuss is distributed to the street illegally. Technically federal law states cannibus is illegal but its state law that allows for its use. State law is trumped by federal law so even someone who is a legit patient can be prosecuted in another state.
