  1. Guest12280544 is correct. The Springfield Model 67H was crafted by Savage Arms, based out of Westfield, Mass.

In addition to this, the guns ranged from Model A to Model H, and since yours is an H (I have one of the same model), it would have been made towards the latter years that the model was in factory. These shotguns were manufactured from 1958 to 1971.

Also, Savage Arms made three different types of these shotguns: Savage (top of the line), Stevens (middle grade), and Springfield (low grade). The Stevens and the Springfield, though apparently cast of the same metal grades, had stocks that were less than great, or so I've read.

The price range for this model in "good condition" is roughly $90-120, though I've seen an "excellent" condition selling for about $385.

Parts are difficult to come by, compared to other models, at least, since they were discontinued, however should you need to assemble/dissemble the shotgun, they are the same build as a Model 30. You can find the instructions in one of the guidebooks available on the Model 30, which you can get for roughly $5 used.
