  3. Johnson was the Vice President of John F Kennedy, and became President due to the assasination by Republicans.

    Hubert Humphrey became his VP during his second term.  

  4. He initially didn't have a VP because there was confusion in the Constitution on this at the time. It was because of this that the 25th Amendment was passed and ratified, which clarified that the President shall nominate someone to be Vice President if that office becomes vacant for what ever reason.

    When he ran for reelection in 1964, he did finally have a running mate, Hubert Humphrey.
