I believe you are talking about Nurse Mildred Ratchet, who was a character in the 1962 novel written by Ken Kelsey.One Flew Over the Cuckoos Nest. It was later made into a film in 1975- Staring Louise Fletcher and Jack Nicholson.

She was a sadistic and horrible person, she was the head administrative nurse at the Salem, Oregon State Mental Hospital.

If you have not seen this movie-I saw it at the drive-in, it is one of the best movies I have ever seen. A real classic, the book is just as good.

I would suggest renting the movie, like I said it is great and is a classic.
