  1. Craig McTavish.

    Although, under one of the collective agreements (I think the one before the one they're using now), a loophole would have allowed players to ditch the helmet. A couple threatened to do so but team management wasn't about to let that happen. I think one or two veterans took them off in the pre-season.

Craig Mactavish formerly of the Philadelphia Flyers.  Got to play without a helmet because the mandatory helmet rule came out after he was an active NHL player.  So the helmet rule didn't apply to him or rather his optional helmet clause was "grandfathered" into his remaining days as a hockey player.

  5. Craig MacTavish was the last NHL player to play an NHL game without a helmet. He retired after the 1996-97 season and is currently the head coach of the Edmonton Oilers. He was only allowed to play without a helmet due to a grandfather clause in his contract.
