  1. They were called "guilds" and they were the predecessors of today's Building Trades Unions.  Just like today, the knowledge, skills, and techniques were learned by young men who serve an apprenticeship carrying tools for, and helping out the Journeymen who execute the plans of the Masters.

  2. Not an easy question to answer.  Most of the great cathedrals were built over many years - sometimes even generations.  This means that there was never one mind or team of minds behind their construction - different people were responsible for different stages of construction.
