  1. The mother or father will sign the birth certificate...however, because the child is a minor you are required to get the parent who didnt sign it to sign a section of it in front of a notary public. There are two forms the passport form and the underage child form and that is where the other parent signs in front of a notary public. Now, if you dont know where the parent lives or how to get a hold of them then you have to notorize that section declaring you do not know where the father is in order to get permission for the passport...confusing but that is what I was told I have to do when my daughter is born.

    You sign all legal documents for the child until they reach the age of 18; at that time they become an adult and can sign for themselves.

    In the application form, in the box provided for applicant's signature put your baby's thumb impression; left thumb if it's a boy, right if it's a girl. Further, the guardian's (father, or mother if father is not alive or in case of divorce) signature is required in the form.
