Who would like to see an example of the "Letter of Non-Impediment to Marriage"? (American marrying Ukrainian)

I have a blank copy that I saved as a pdf. (or some kind of format of pdf?) and would like to share it with any U.S. citizen looking into getting Married in Ukraine.

Last year I was looking all over the net for an example of

this "Letter of Non-Impediment to Marriage" and was never able to find one, but now I have one and would like to share it. (It's just basic information that you need to fill out)

Letter of Non-impediments to Marriage simply shows that a person had no bar to marry such as not presently married to any person.

The correct, most current required  format of this letter is available at the American Citizen Services section of the US Embassy in Kiev.  Since someone intending to marry a Ukrainian citizen has to go to ACS anyway to have the letter notarized, it can be filled out there.
