Whose fault is it if you're driving on the fwy, switch lanes to the left and get hit by a car-driver's side?

The person changing lanes.  It's their responsibility to make sure the lane is clear.  Relying on mirrors is not enough, they have blind spots.  You have to turn your head and look.

If they changed lanes and hit you, they are at fault.

If it was you who changed lanes... um... sorry, your fault.

you are at fault.  they were in there first, and you failed to check your blind spots or whatever to make sure the coast is clear.
