If you mean metaphorically, it's because he's been told since he could walk that he was special and destined for great things, without being told of the responsibilities that come with that.  If you mean literally, it's believed to be the result of steroid use.

I have wondered that myself, haha!! When you see pictures of him when he first started playing professionaly his entire body is smaller. Now he is huge everywhere. Especially his head though! I believe he has abused steroids. He also has a big head because his head is filled of thoughts of how great and amazing he is. I am surprised it has not popped off!
