  1. you probably dint give him the right food tadpoles eat algae so do young frogs

  2. all frogs croak eventually

  3. did he have food to eat?

maybe the temperatures were too hot or too cold?

    there are alot of factors.

    sorry about your froggie..

  4. He possibly drowned, a lot of people don't realize that a lot of frogs don't actually live in water as frogs...however, one inch of water isn't much, especially compared to how large leopard frogs usually are even right after metamorphasis.

You didn't say what you were feeding it.  Possibly the food was too big (unlikely-frogs can eat something their own size) or contaminated with something.  Crickets dusted with mineral and vitamin supplement usually is agood start.

Was the rock a "hot" rock?  If so, it probably got burnt.

The most likely culprit in my mind is the water.  How often is it changed?  If not filtered, a partial change everyother day is sometimes needed-but it should be at least once a week.  Also, did you use a purifier to remove clorine, cloramine, and other contaminants?  A filter or just aging water doesn't quite cut it.
