Why did the duck egg die?

Question

i know my mom had it on a high temperature for a while but i am wondering if there is another reason. i put water on it a lot to keep the humidity high. i heard that may be bad. if i tried to hatch a new egg would it die from the water? and how to i keep the tank humid, because it is just a glass tank with no top.
