  2. I don't know, maybe call them and ask them?

I just got mine on Friday, they asked me.. and I'm 17.. (Pennsylvania)

  3. Get yourself a seperate donor card to carry around in your wallet or with your counterpart licence.

  4. They didn't ask me that either in California. I didn't check it on the form. They must have assumed I can read and comprehend.

  5. It might be because you aren't 18, but I don't know for sure because I'm not from Illinois.

The one thing you can do is sign up on your state's organ donor registry.  It won't be valid until you turn 18, but if the chance to be a donor came up, it would let your family know your wishes.
