Why do both my phone and camera say that the format for my SD memory card is incorrect?

I have a microSD card adapter for my camera, and when i plug my camera in, it says its the incorrect format. Also, when i take the microSD card and put it in y phone (where it used to work), the phone says its an incorrect format and asks me if i want to reformat it. When i say yes, it says there was an error. What do i do about this?

I suggest taking out all the important files out of the micro sd and saving them on your computer. Format your sd card. Then put your files back in the card and try it out.
