  2. I'm not sure but some I have seen some commercial toilet seats without a gap and some residential ones with a gap. Good question. :)

  3. The gap is required by some building and sanitation codes in the US. The gap is said to make it more sanitary and easier to keep clean. Especially in the men's room.
