Why do i not recieve my xbox 360 achievments? ie gow cog tags, i have all 30 but no achievment, same for proev

in gow i have all 30 cog tags but i havnt recieved the achievment. In pro ev i completed a league undefeated and never recieved the achievment.

Why does this happen & how do i resolve it?
