Because it's nearly universal in the western world that pointing to a watch implies a clock, meaning time.

Pointing to your hoo-hoo is not so clear-cut.

"I have to pee."

"This is my v****a".

"Please do me."

"It has weird bumps."

It'd be funny to see people's reactions though.
