I'm not sure if we are all understanding what you are asking.  I may be wrong but I don't think you are asking why the shoes are removed by emergency services people, I think you are referring to the fact that accident victims are often knocked right out of their shoes.  That is a fact that happens all the time with pedestrian accident victims.  It's a matter of the high level of force applied to a human body when struck by a vehicle.  I can't give you a technical reason, but it is very common occurrence.  You often see news photos of a smashed up bicycle with someones shoes lying nearby.  I hope I answered your question, but if the other answerer's correctly figured out what you meant then I know they did a good job.  Cheers!!

Part of the physical exam performed by EMS personnel for trauma patients is to expose the body to visually inspect it. They need to expose the feet to check for-

- Sensation: can the person identify which toe is touched

- Movement: can the person wiggle their toes

- Circulation: status of the Dorsalis Pedis Pulse (located on the top of the foot)
