  1. It could be one of two reasons.  First, your insurance is a PPO plan and the dentist 35-40 miles away is in network with your plan.  Or second possibility is, it's your choice to drive that far in order to be treated by a high quality provider.  If you are limited to who you can go to due to your insurance, the plan you have probably is not a good one for the dental providers in your area.  Many in my area have such low reimbursement, that I would never sign up for them no matter how many patient's are in my geographic area.  Bottom line, it's a business decision each practitioner makes.
