  1. The nickname "sawbones" has been used for doctors for years.


  2. because he is a doctor. it is a nick name.

  3. For the same reason that Seeley Booth calls Temperance Brenner "Bones."  Sawbones has been a nickname for surgeons, and by extension all doctors since the US-Mexican War of 1846-8.
