  1. I have similar experience I tried 3 headphones and one of them mute the speakers and the others don't.

    The quality of the  jack might cause contact problems  and might not mute the speakers. ( assuming that your equipment has auto muting of the speakers when the headphones are connected)

    Answer: probably because the device you're plugging your headphones into doesn't have an automatic speaker-mute switch built in.  You will have to manually switch off the speakers.  

    More than likely somewhere near the POWER button on your stereo or surround sound receiver are a pair of switches labeled "Speaker A/B" - turn those off when you plug your headphones in and that should do it!

    Just remember to re-activate your speaker(s) with the same button(s) after you're done listening with headphones!

  3. You don't give much detail so it's hard to be definite.

    Some equipment turns off speakers when headphones are plugged in, while others do not (you have to actually turn off the speakers with a switch).

    If speakers turn ON when headphones are plugged in something is wired wrong.
