Why hasn't my blood pressure gone down yet?

About a month ago i was having real bad headaches. went to the dr. and found my bp at 190/128. had a doppler done on my neck and found 50-69% blockage in the left carotid artery. he put me on Plavix 75 mg 1x a day and Carvedilol 3.125 mg 2x a day. my blood pressure came down to 135/89.

now it is back up to 189/110. He doubled my Carvedilol. I started yesterday. But it is still 182/112 !!

is it too soon to see a change?

  1. Ok the Plavix will just prevent you really from having a stroke or heart attack.  The Coreg (Carvedilol) is for blood pressure.  You are still on a low-ish dose.  Coreg goes up to 25mg tablets, so there is still room for you.  Also blood pressures tend to be much more high on home blood pressure machines.  Unless you have someone with a manual cuff doing your blood pressure for you, I wouldn't rely on anything but what a doctor or nurse would tell you regarding your blood pressure.  My grandfather's blood pressure always read 180's over 90's.  I'd take his blood pressure myself and it would be 140's over 70's.  Big difference.  It can take up to 2 weeks for the effects of Coreg to really be noticed and dosages shouldn't be increased more frequently than every 10 days.  Even then the recommended starting dose is 6.25mg twice a day and worked up every 2 weeks until the desired blood pressure is achieved.  

    However you need to watch your heart RATE.  Coreg will slow the heart down as well as drop blood pressure.  If your heart rate goes below 65 or 60, call your doctor.  You may end up needing a different blood pressure medication.  I'm surprised that they didn't put you on Clonidine (Catapress) 0.1mg which has a very immediate reaction with blood pressure.  I'd be more concened about that blockage.  You may need an angioplasty to help widen those carotids.  At the worst you'd need a carotid endarectomy, but it's risky.   Just keep going on the Coreg and continue to monitor your blood pressure.  Take it right before you take your medication and then an hour afterward-  you should see some difference in it.


  2. A blood pressure(BP) of 190/128 is to be brought down to normal immediately and is to be maintained at the normal levels only. As you are already under the treatment of your doctor he may change the anti hypertensive drugs now again until your BP is controlled at normal levels.

    You may please follow your doctors advice.
