  1. Age if the first factor considered. Smoking is the second factor. Occupation is the third, but location is always a factor. The location factor is called the "Differential". It takes into account the mortality rates of that state and location as pertains to the coverage. You didn't say how many are covered under your policy, as well as how much the policy will cover. There are many types of health coverage from major medical two million to five in the case of humana versus a supplemental coverage.

  2. Different coverage, different states, different risk factors.  Any number of reasons.

  3. I'm guessing you are talking about the Humana Medicare Advantage plans.

These plans are county specific and the premium can vary by county for nearly identical plans.

The difference in premium could be a combination of the following:

Different plans (Humana has HMO, PPO, & PFFS plans) which have slightly different coverage and co-pays.

The different plans pay the doctors different. A doctor accepting an HMO gets paid less but more often than a doctor accepting a PPO. A doctor accepting the PFFS gets paid the most.

Different drug co-pays and coverage through the donut hole.

The number of people on each plan.

The experience they have paying claims - if more people use the plan more often in one area the premium will be higher because Humana pays out more in claims.

Additional benefits - some plans will have limited dental, vision, and other benefits.
