If your oxygen level drops then the skin under your nails will discolor. Nail polish will hide this fact and the docs won't be able to see it.

Because during surgery your oxygen saturation levels will be monitored. This is done using a sensor on the fingernail. Nail polish can interfere with this monitoring.
